MINOR ATHLETE PARTICIPATION WAIVER AND CONSENT & CODE OF CONDUCT

A parent or guardian must read and agree to the following documents in order for their U18 athlete to participate in USDBF activities, including receiving email communcations from the USDBF. The Code of Conduct applies to all USDBF members. The Minor Athlete Participation Waiver and Consent document only applies to our members who are 18 years of age or younger.
